Join us and become part of a team dedicated to making a difference every day.Responsibilities:Provide nursing care in accordance with resident assessments, care plans, and facility policiesAdminister medications, treatments, enteral feedings, and IV therapies within the LPN scope of practiceMonitor resident conditions and promptly communicate changes to providers, supervisors, and responsible partiesAccurately document care, treatments, observations, and medication administrationSupervise and support Nursing Assistants to ensure quality resident careMaintain up-to-date resident care information and communicate pertinent information during shift changesFollow infection control, safety, resident rights, and confidentiality standardsParticipate in interdisciplinary care planning and quality improvement initiativesAssist with admissions, transfers, and other nursing responsibilities as assignedRequirements:High School Diploma or equivalent required; Associate's Degree preferredValid NYS Licensed Practical Nurse (LPN) License requiredCurrent CPR Certification requiredExperience in a skilled nursing facility or with the geriatric population preferredStrong clinical, communication, and organizational skillsKnowledge of medication administration, infection control, and resident care practicesAbility to supervise nursing assistants and work effectively as part of an interdisciplinary teamCompassionate, resident-centered approach to careBenefits:Paid Time Off (PTO) for Full-Time and Part-TimeWeekly or Same-Day Pay OptionsHealth, Dental, and Vision InsuranceLife InsuranceEmployee Referral Bonus ProgramRetirement Saving OptionsPreceptor Bonus Program (CNA & LPN)
