To enhance patient care within diabetes and obesity management, the part-time Louisiana Licensed Clinical Pharmacist will provide medication therapy management, collaborate with healthcare teams, and conduct clinical assessments while working remotely. Key responsibilities Provide medication therapy management, including medication reconciliation and adherence counseling for patients with diabetes and obesity Collaborate with interdisciplinary care teams to develop individualized care plans and ensure compliance with clinical guidelines Monitor medication regimens and participate in quality improvement initiatives to enhance clinical outcomes and medication management Required qualifications Doctor of Pharmacy (PharmD) and a valid pharmacist license in Louisiana Minimum of 3 years of experience as a clinical pharmacist; PGY1/PGY2 residency preferred Strong clinical knowledge in diabetes care and disease state management Proficient in Microsoft Office tools and data management Ability to maintain confidentiality and handle sensitive information with discretion
