Community Health Worker Project DirectorLocation: Nashville, TNDuration: 12+ MonthsJob Summary:The Community Health Worker (CHW) Project Director position will be responsible for managing and coordinating projects and initiatives related to Community Health Workers for the Tennessee Department of Health (TDH), Division of Health Disparities Elimination. Must live in the State of Tennessee.Key Duties/Responsibilities:Provide coordination of CHW initiatives across the division and the departmentPlan and convene meetings with internal and external partners that support, hire, or train CHWsServe as a subject matter expert on CHW infrastructure sustainability efforts and best practicesStrengthen community-based organization capacity as health system partners and CHW employersLeverage existing resources and establish new partnerships that support the CHW workforce in TennesseeSupport collaboration and strategic planning that integrate and align CHW efforts into broader statewide workforce effortsSkills and Abilities:Excellent presentation performance skillsClear and concise communication skillsPossess patience and professionalismDemonstrated ability to collaborate effectively in a team settingDemonstrated ability to work independently and with a teamStrong ability to maintain effective and professional relationships with trainees, team members, and community partnersStrong in-person and virtual facilitation skillsProficiency in Microsoft Office SuiteEducation and Experience:Bachelor’s Degree in Social Work, Human Services, Healthcare, Public Health, or related field. Four (4) years of relevant community health worker, community outreach, or health education experience may be considered in lieu of degree.
