Long Term Care Functional Screener

Role: Long-Term Care Functional Screener responsible for assessing eligibility for publicly funded long-term care services within the IRIS Program.

Key Responsibilities

• Build trusting relationships with IRIS participants and provide guidance to maintain funding eligibility.
• Administer and coordinate Long-Term Care Functional Screens (LTC FS) and renewals.
• Collaborate with team members and community partners, participate in meetings, and prepare reports.
• Ensure documentation compliance with federal and state standards and support community connections.

Qualifications & Skills

• Bachelor’s degree in social work, psychology, human services, nursing, or related field.
• At least 1 year of experience working with seniors or individuals with disabilities.
• Valid LTC FS Certification (or willingness to obtain).
• Strong organizational skills, familiarity with technology, and a commitment to participant autonomy.
• Bilingual skills are a plus.

Requirements

• Valid driver’s license, reliable vehicle, and satisfactory driving record.
• Ability to pass background checks and willingness to travel frequently.
