Long-Term Care Functional Screener
Responsibilities
Build trusting relationships with IRIS participants, administer and coordinate LTC Functional Screens, document renewals, provide guidance to maintain funding eligibility, collaborate with team members and community partners, participate in meetings, prepare reports, testify in hearings, ensure compliance with documentation standards, and strengthen community connections.
Qualifications
Bachelor's degree in social work or related field, minimum 1 year experience with seniors or individuals with disabilities, valid LTC FS certification or willingness to obtain, bilingual skills a plus, strong organizational and communication skills, proficiency with technology, and the ability to work independently and collaboratively.
Requirements
Valid driver's license, reliable vehicle, satisfactory driving record, pass background check, and willingness to travel frequently.
