We are seeking an experienced NursePractitioner or Physician Assistant for aLocums at a primary care practice serving geriatric patients inavalue-based care model.This is a hybrid role with blend ofcoverage in outpatient clinics as well as home-based patient care.
Compensation: $75 – $95 perhour per day
Position Details:
- Schedule: Full TimeHours Guaranteed
- Monday – Friday - 5 Days Per Week
- 8:30 Start Time
- Patient Load: LOW DAILYCENSUS
- Home Care: 3–5 patientsper day
- Clinic: Small census of 7–10 patients perday
- Setting: Hybrid (Outpatient clinic +home-based care), value-based primary care for elderly patients
Responsibilities:
- Deliver comprehensive primary care services,including preventive care, chronic disease management, and acute caretoGeriatric patients
- Diagnose, treat, prescribe medications, anddevelop individualized treatment plans
- Order and interpret diagnostic tests (labs,imaging, etc.)
- Document accurately in the EHR system
- Provide patient education and promotewellness initiatives
- Collaborate with physicians, nurses, andsupport staff to ensure coordinated, high-quality care
- Refer patients to specialists and managefollow-ups
- Ensure compliance with state, federal, andprofessional regulations
Requirements:
- Active NP or PA license in NY
- Minimum 2 years of primary care experience
- National Board Certification
- Expedited Credentialing
- Medicaid and Medicare Provider Id required
