• Board Certified Urology - Preferred. Will accept Board Eligible providers if they are within 5 years from residency
• North Dakota License - Highly Preferred
Avg callback: 1-5 hours
Setting: Clinic and Call
• Duties: Clinic and Call duties
• FTE:1.0
• Required Procedures: cystoscopy, cystoscopy and dilation
• EMR: EPIC
• Support Staff: 3 RN's, 1 CNA
• Timeframe: 90 days
• Requirements: completed credentialing application, ND license, DEA license, supplemental documentation, case logs
