Qualifications

• Board Certified in Obstetrics and Gynecology (No Board Eligible Candidates)
• Required
• Valid Michigan Medical License and Controlled Substances Registration (CSR) at the time of submission
• Basic Life Support (BLS) Certification - Required
• Provider must be a local resident
• REQUIRED, no exceptions
• Clean background necessary for expedited credentialing - REQUIRED, no exceptions
• Please provide all availability if unable to commit to the specified dates - this is required.
• DEA Registration
