- Conduct new patient exams and hygiene checks
- Administer fillings, root canals, extractions, and dentures
- Provider must be comfortable with Surgical Extractions
- Resume
- Professional License
- Active DEA license is required
- Copy of Controlled Substance Regulation (CSR)
- Copy of Diploma
- Front and Back Copy of Driver's License
- Copy of Malpractice Insurance
