Behavioral Health Licensed Social Worker
The Behavioral Health Licensed Social Worker assesses the patient's psychosocial problems and needs at admission and ongoing thereafter and, in collaboration with the patient, family significant other, and members of the treatment team, develops a plan for treatment and aftercare based upon the identified, prioritized, psychosocial problems.
The Licensed Social Worker acts as primary therapist for assigned patients. - Assesses the patient's psychosocial problems and needs within the required time frame after admission and ongoing thereafter. - Collaborates with the patient, family/significant other, and the others of the treatment team. Carries out the plan for treatment and aftercare as designed, evaluates the patient's responses to the planned interventions, and makes decisions regarding continuation or revision of the treatment plan. -Collaborates with physicians and other members of the health care team to further their understanding of significant social and emotional factors underlying the patient's mental health problems. -Responsible for discharge planning for assigned patients, arranging appropriate discharge plans in collaboration with other members of the health care team. -Advocates for patients in their relationships with other hospitals and outside agencies. -Documents all phases of the treatment process in the medical record within appropriate timeframes. -Maintains accountability for clinical competency, professional growth and development and the professional ethics of clinical practice. -Participates in special projects as requested. -Attends meetings in the community as requested and/or according to professional interest.
