Kaiser Permanente seeks an LMSW Case Manager in our Healthcare & Medical Services department. In this role, you’ll provide psychosocial assessments, develop person-centered care plans, coordinate community and medical resources, and advocate for patients across the continuum of care. You will collaborate closely with an interdisciplinary team to reduce barriers to treatment, support chronic disease management, and improve health outcomes.
Using evidence-based interventions, you’ll offer counseling, crisis intervention, and discharge planning while documenting in the EMR, tracking progress, and ensuring compliance with regulatory and ethical standards.
