Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit.
The individual must demonstrate knowledge of the principles of growth and development across the lifespan.
Possess the ability to assess data reflective of the patient's status.
Interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.
Provide direct and indirect skilled nursing care for assigned residents.
Follow defined policies and procedures under the supervision of a Registered Nurse.
Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
Maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
Collect data relevant to each resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the interdisciplinary team.
Contribute to the creation of a culturally relevant plan of resident care reflecting nursing diagnoses and interventions to achieve desired resident outcomes.
Involve the resident and family in the care process while promoting a healthy and safe environment.
Recommend revisions to the plan of care after evaluating resident outcomes.
Indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
Collaborate with the resident, family, and members of the interdisciplinary team within the scope of nursing practice.
Assure customer satisfaction.
Maintain current knowledge and competency in nursing practice.
Participate in the mentoring and orientation of staff.
Consider factors related to safety, cost, and impact when planning the delivery of care.
Under the direction of the RN, provide leadership in the professional practice setting.
Demonstrate knowledge and competency in nursing practice.
Occupational exposure to bloodborne pathogens is reasonably anticipated.
Required tasks involve potential contact with blood, body fluids, or tissues.
