Providing holistic, evidenced-based care, the full-time Licensed Virtual Care Nurse Practitioner will deliver patient services remotely, including assessment, diagnosis, treatment, and health counseling for patients aged 18 months and above, with a schedule that includes every other weekend. Key responsibilities: Deliver accurate assessment, diagnosis, treatment, and management of health problems while providing health counseling and disposition planning Evaluate primary care and transitional care patients, offering healthcare education and counseling Document patient care within an EHR and coordinate care with internal and external colleagues to ensure high standards of care Required qualifications: Current unrestricted license in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV Minimum of two years of medically relevant experience or equivalent Completion of a Master's Degree level Family Nurse Practitioner program with current National Board Certification Ability to work autonomously and collaborate effectively with healthcare teams Proficiency in information management and technology
