Job Title: Licensed Practical Nurse (LPN) - Home Health
Position Summary
The Licensed Practical Nurse (LPN) provides direct, hands-on nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The LPN supports patient independence, safety, and comfort while following individualized care plans and agency policies.
Essential Duties and Responsibilities
  • Provide skilled nursing care to patients in their homes according to the plan of care
  • Monitor and document vital signs, symptoms, and overall patient condition
  • Administer medications and treatments as prescribed
  • Perform wound care, catheter care, ostomy care, and dressing changes
  • Assist patients with activities of daily living (ADLs) as needed
  • Observe and report changes in patient condition to the supervising RN or physician
  • Provide education to patients and caregivers on disease management, medication use, and safety
  • Maintain accurate, timely clinical documentation in accordance with regulatory requirements
  • Follow infection control, safety, and confidentiality standards
  • Collaborate with interdisciplinary team members to ensure coordinated care
  • Participate in case conferences, in-service training, and quality improvement activities
Qualifications
Education and Licensure
  • Graduate of an approved Practical Nursing program
  • Current LPN license in the state of practice
  • CPR certification (BLS)
Experience
  • Previous home health or clinical nursing experience preferred
  • Experience with geriatric or chronic care patients a plus
Knowledge, Skills, and Abilities
  • Knowledge of basic nursing principles and home health practices
  • Ability to work independently and make appropriate clinical decisions
  • Strong assessment, communication, and documentation skills
  • Compassionate, patient-centered approach to care
  • Ability to follow care plans and physician orders accurately
  • Basic computer skills for electronic health records
  • Reliable transportation and ability to travel to patient homes
Working Conditions
  • Work performed in patients' homes and community settings
  • May require travel, flexible scheduling, evenings, weekends, or holidays
  • Ability to lift, reposition, and assist patients as needed

Reports to: Registered Nurse / Clinical Supervisor
