Licensed Practical Nurse (LPN) | Care Delivery
Location: Springfield, MA
Pay: $35 to $37 per hour
Schedule: Monday through Friday, 8:30 AM to 5:00 PM
Hours: 40 hours per week
Employment Type: 6-month contract-to-hire
Work Environment: Fully onsite in a clinical office setting

About the Opportunity
We are looking for a compassionate and dependable Licensed Practical Nurse to join an onsite clinical team in Springfield. In this position, you will provide hands-on patient care, support providers during appointments, and help patients navigate follow-up care, prescriptions, and other healthcare needs.
This is a great opportunity for an experienced LPN who enjoys working directly with patients while also handling clinical coordination and administrative responsibilities. The position begins as a six-month contract with the potential to transition into a permanent role.
What You'll Do
- Take patient vital signs and document medical histories
- Prepare patients for exams and assist healthcare providers
- Perform blood draws and collect clinical specimens
- Administer immunizations and medications
- Provide basic patient care, including wound care
- Process laboratory samples and document results
- Support prescription refills and renewal requests
- Assist with prior authorizations
- Schedule appointments and make patient follow-up calls
- Maintain accurate and timely patient records
- Provide occasional telehealth support
- Deliver a welcoming and professional patient experience
What We're Looking For
- Active and unrestricted Massachusetts LPN license
- At least two years of related clinical experience
- High school diploma or GED
- Current CPR certification
- Hands-on experience with phlebotomy, immunizations, medication administration, and basic wound care
- Strong patient service and communication skills
- Ability to work independently and collaboratively
- Basic proficiency with Microsoft Outlook, Word, and Excel
- Ability to meet the physical requirements of the position, including occasionally assisting with or moving more than 100 pounds
Preferred Qualifications
- Associate degree in Nursing
- Experience working in a managed-care environment
- Familiarity with Medicaid or Medicare populations
- Telehealth experience
- Experience with prescription renewals or prior authorizations
- Strong documentation, critical-thinking, and time-management skills
