nResponsibilities:
nAdminister medications and treatments as prescribed by healthcare providers
nMonitor and document patient vital signs and overall health status
nAssist with daily living activities such as bathing, grooming, and meal preparation
nEducate patients and families on managing health conditions and care plans
nCoordinate with RNs, therapists, physicians, and other healthcare professionals
nMaintain accurate and timely documentation using EHR systems
nProvide emotional support and companionship
nFollow all safety and health regulations in home care settings
nRequirements:
nActive and valid LPN license in Massachusetts
nMinimum 1 year of experience in home health or related healthcare setting
nCurrent BLS certification
nReliable transportation and willingness to travel to patient homes
nProficiency in electronic health records (EHR)
nStrong communication and interpersonal skills
nCompassion and commitment to quality patient care
