Job Description

Day Shift 8 hours/day 5 days/week

Title: LVN / LPN
Duration: 13 weeks
Location: 100 Kings Hwy S Rochester, NY 14617
Schedule: Day Shift 5x8-Hour (08:00 - 16:30)

  • Position: LPN – Care Manager
  • FTE: Full Time | Guaranteed: 40 hrs/week
  • Location: RGMG Care Managers – Riedman Administrative Campus
  • Role Type: Mobile / Remote / Hospital-based Care Coordination
  • Floating: Rochester General Hospital, Unity Hospital, patient homes & provider offices
  • Specialty: Adult/Geriatric
  • Experience: 1+ year; Home Health/Mobile Case Management preferred
  • License/Cert: BLS required
  • Locals: Accepted; lower rate if within 75 miles
  • First-time Travelers: Accepted
  • Support Transitions of Care program focused on reducing 30-day readmissions.
  • Work under RN Care Manager to implement the Plan of Care (POC) .
  • Telephonic/remote patient screening, follow-up, triage, education and care coordination.
  • Schedule appointments and identify/resolve barriers to follow-up.
  • Monitor biometric data and medication adherence.
  • Complete home, hospital and provider-office visits as needed.
  • Connect patients with community resources.
  • Participate in virtual multidisciplinary care-team rounds with RN, Social Worker, Pharmacist, etc.
  • Maintain frequent patient contact and document all activity in the EHR .
  • Manage approximately 25–30 active patients at a time; 30–40 rotating patients/month.
  • Program runs approximately 30 days , followed by reassessment/closure or transition to long-term management.
  • Reliable transportation required; LPN does not transport patients.

Preferred:

  • CHF, COPD & complex comorbidity experience
  • Community resource knowledge
  • Strong communication, documentation and independent-work skills
  • Flexible with daily travel/assignment changes

Key Point: This is primarily a Care Coordination / Case Management / Transitions of Care role, not bedside or hands-on nursing.
