Licensed Practical Nurse

To support patients enrolled in chronic care management and remote patient monitoring programs, a full-time Care Navigator will leverage their New York Licensed LPN credentials to educate and empower patients while maintaining accurate documentation and collaborating with healthcare teams remotely. Key Responsibilities: Complete phone consultations with patients, providing support and education about their chronic conditions Educate patients on health conditions and assist them in setting and achieving goals aligned with their care plans Maintain accurate patient records and respond to remotely transmitted patient data according to established workflows Required Qualifications: Current, valid, and in good standing New York Nursing License (LPN/LVN) 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management Proficiency in using electronic health records (EHR) and care management software Reliable internet connection and proficiency with virtual communication tools Empathy and a patient-centered approach to care
