What You'll Be Doing
As a Bilingual Patient Support Specialist (LPN), you'll provide hands-on clinical support while helping patients access the care and resources they need.
Responsibilities Include:
- Deliver patient-centered nursing care in an ambulatory primary care setting
- Conduct proactive patient outreach and engagement to improve continuity of care
- Coordinate care plans and assist patients in navigating healthcare services
- Respond to patient phone calls and electronic messages while escalating concerns when needed
- Perform clinical testing and nursing procedures within LPN scope of practice
- Support primary care and specialty care programs
- Maintain exam rooms, clinical supplies, and medical equipment
- Accurately document patient care in electronic health records (EHR)
- Participate in population health and quality improvement initiatives, including UDS and HEDIS measures
- Ensure compliance with HIPAA, OSHA, CLIA, HRSA, and Joint Commission standards
- Collaborate with providers and interdisciplinary care teams to deliver exceptional patient outcomes
Qualifications
Required:
- Graduate of an accredited Licensed Practical Nurse (LPN) Program
- Active Connecticut LPN License
- Current CPR/BLS Certification
- 2-4 years of clinical experience in: Primary Care, Ambulatory Care, Acute Care
- Strong care coordination and patient communication skills
- Experience with Electronic Health Records (Epic experience preferred)
- Ability to thrive in a fast-paced healthcare environment
- Bilingual in Spanish and English
