Qualifications:
- Education: Completion of an accredited Licensed Practical Nursing (LPN) program.
- Licensure/Certifications: Must have an active New York State LPN license.
- Experience : Must have at least two (2) years of experience in a nursing home or hospital setting.

- LPN Registration
- Driver's License or Passport or Green Card and SSN Card
- COVID Vaccination Card
- MMRV Results (Mumps, Measles, Rubella, and Varicella)
- Physical Assessment
- CPR
- Must be immune to Hep B
