Registered Nurse (RN) / Licensed Practical Nurse (LPN)Registered Nurse (RN) / Licensed Practical Nurse (LPN) is responsible for performing essential care for our members in our nursing supported group homes under the direction of the Chief Nursing Officer and NSGH Administrator. This is a safety sensitive position.Essential functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Responsibilities for Members which includes:Provide for the safety, emotional and physical needs of all membersEnsure members are clean, well-groomed and dressed age appropriately and correctly for the season and environmental conditionsTransportation to and from day program as neededPosition and re-position every 1 hour while awake and every 2 hours while asleep and as neededHelp members cook meals and to eat as neededEnsure daily that members are being emotionally and physically stimulated to the best of their abilityAssist with showering members to ensure a complete shower is given as neededResponsibility for Medical Aspects of Care which includes:Responsibility to the Nursing Supported Group Home Manager and Chief Nursing Officer for reporting all medical aspects of care and changes in conditions regarding the members in the homePerforming routine nursing duties, including:Taking a recording temperature, pulse, and blood pressureComfort, grooming, and hygiene needsDiet managementCollecting specimensPatient activitiesPassing medicationsReordering medications as needed (or ensuring Nursing Services Director is aware and needs to reorder)Assisting in direct care of members to include, but not be limited to:Showering membersChanging/Repositioning membersCooking mealsAssisting in cleaning the homeAssisting in doing laundrySafeguard equipment and supplies, and checks equipment apparatus or appliances furnished to resident in the course of treatmentMaintaining and ensuring that all documentation is completed before end of shift which includes but not limited to: incident reports, daily nursing notes, Med Sheets, etc.Responsibility to the Company which includes:Being flexible in hours worked and location worked in order to assist with staff coverage if neededEnsuring that you maintain compliance in regard to:Article 9Prevention and SupportFingerprint Clearance CardCPR/1st AidAdhere to and follow all directives and policy and procedures handed down by the companyEnsure that all required documentation is completed before the end of shift.Other Duties: Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job. be able to understand what is being said to him/her in English. Must be able to use the English language so that what he/she is saying to another person can be understood, in all emergency situations so that the safety of patients/clients, other employees and other individuals who are in our facilities conducting is not jeopardized.
