Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Provide skilled nursing care under the direct supervision of the clinical supervisor
- Complete evaluation tasks including vital signs and medication reviews
- Coordinate with physicians, therapists, and other individuals in the client's care plan
- Assess and chart observations of the patient's condition at each visit
- Maintain complete and accurate documentation of nursing care
- Work collaboratively with all members of the health care team
- Provide education to patients and families on proper home health care procedures and strategies
- Record patient's medical information and vital signs
- Monitor, record, and report symptoms or changes in patient's conditions
- Consult and coordinate with health care team members to assess, plan, implement, or evaluate patient care plans
- Administer local, inhalation, intravenous, or other anesthetics
- Prepare patients for and assist with examinations or treatments
- Modify patient treatment plans as indicated by patient's responses and conditions
- Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition
- Monitor all aspects of patient care, including diet and physical activity
- Assess the needs of individuals, families, or communities, including the assessment of the patient's home or work environments, to identify potential health or safety problems
- Engage in research activities related to nursing
- Perform other duties as assigned
Minimum Qualifications (Knowledge, Skills, and Abilities)
· Associates degree required; Bachelor's degree from school of nursing preferred
· Current and valid Licensed Practical Nurse license
· One year of LPN experience
· Current CPR certification required
· Valid driver's license required
· Current automobile registration and insurance required
· Strong interpersonal skills that build personal rapport and establish trust with the patient
· Computer literate
· Strong organizational skills
· Excellent written and communication skills
· Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
Requirements
- Valid LPN license, Prior outpatient or clinic nursing experience preferred & Strong clinical and communication skills
