About the Role

Responsibilities
- Provide direct nursing care to long-term care residents.
- Administer medications and treatments as prescribed.
- Monitor, assess, and document resident conditions.
- Collaborate with the interdisciplinary team to deliver quality care.
- Float within the facility as needed to support patient care.
- Active New York LPN License .
- Current BLS Certification .
- Minimum 1+ year of Long-Term Care (LTC) experience.
- Experience with MS, Orthopedic, Dementia, and Memory Care patients preferred.
- PointClickCare EMR experience preferred.
- Must provide NURSYS verification and valid driver's license matching demographics.
- Strong communication, teamwork, and patient care skills.
