About the Facility - Palo Alto VAMC

VAPAHCS is a teaching hospital associated with the Stanford University School of Medicine. It provides patient care services as well as education and research. Health care is provided in areas of medicine, surgery, psychiatry, rehabilitation, neurology, oncology, dentistry, geriatrics, and extended care. 1,300 University residents, interns, and students are trained annually.
VAPAHCS operates nearly 900 beds, including three nursing homes and a 100-bed homeless domiciliary serving more than 85,000 enrolled veterans.

LPN - Long Term Care - RESPONSIBILITIES (listed are included but not limited to)
  • The LPN working in the Community Living Center shall provide services supporting the medical staff (physicians and registered nurses) in the long-term care division, which is comprised of 3 long-term care units: long-term, short-stay, and hospice/palliative care residents.
  • Shall be responsible and accountable for all elements of the nursing process when providing and/or supporting direct patient care.
  • The LPN - CLC shall assess plans implements and evaluates care based on age-specific components based on the directives of their supervisors.
  • Shall be responsible for patient activities of daily living, assessments, medication, administering medication and procedures per established policies and guidelines, ion administration, IV therapy, interdisciplinary team coordination, admissions, and discharges, internal and external transfers, delegating to assistive personnel, electronic medical record documentation , patient transport, and emergency response procedures.

LPN - Long-Term Care - REQUIREMENTS
  • A current license as a practical nurse/vocational nurse in at least one state, territory, or commonwealth in the US or the District of Columbia.
  • A minimum of one year direct patient care experience.
  • Must have Basic Life Support certification.
  • Must be capable of assisting the patients in setting up, standing, and transportation.
