Position Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care and health support services to participants enrolled in the Adult Medical Day Care (AMDC) program. The LPN works under the supervision of the Registered Nurse (RN) and interdisciplinary team to promote participant health, safety, independence, dignity, and quality of life in accordance with Maryland Department of Health regulations, Medicaid requirements, and facility policies.

Essential Duties and Responsibilities

Clinical Nursing Services

Provide direct nursing care to participants according to individualized care plans.

Monitor participant vital signs, blood glucose levels, weights, and other clinical indicators as ordered.

Observe and report changes in participant condition to the RN, physician, and interdisciplinary team.

Assist with medication administration and treatment procedures in accordance with Maryland nursing laws and AMDC policies.

Perform treatments including wound care, catheter care, skin assessments, and other nursing procedures within scope of practice.

Maintain infection prevention and control standards throughout the facility.

Respond appropriately to participant emergencies and medical incidents.

Participant Care Coordination

Assist with development and implementation of participant service plans.

Participate in interdisciplinary team meetings and care conferences.

Communicate participant needs and concerns to family members, caregivers, physicians, and team members as directed.

Support participant rehabilitation, restorative care, and wellness activities.

Documentation & Compliance

Complete accurate and timely nursing documentation in participant records.

Document medication administration, treatments, incidents, and participant responses.

Maintain compliance with Medicaid, HIPAA, OSHA, OHCQ, and Adult Medical Day Care regulations.

Report incidents, accidents, abuse, neglect, or safety concerns immediately according to facility policy.

Safety & Infection Control

Follow universal precautions and infection control procedures.

Maintain a clean, safe, and organized clinical environment.

Ensure safe handling and storage of medications and medical supplies.

Participate in emergency preparedness drills and safety training.

Additional Duties

Assist with participant admissions, transfers, and discharges.

Supervise Certified Nursing Assistants (CNAs) and support staff as assigned.

Attend required staff meetings, trainings, and continuing education programs.

Perform other duties as assigned by nursing leadership.

Qualifications

Education

Graduate of an approved Licensed Practical Nursing program.

Licensure

Current active Licensed Practical Nurse (LPN) license in the State of Maryland.

Certifications

Current CPR/BLS certification required.

First Aid certification preferred.

Experience

Minimum one (1) year nursing experience preferred.

Experience in adult day care, geriatrics, long-term care, rehabilitation, or community health preferred.

Knowledge, Skills, and Abilities

Knowledge of adult medical day care regulations and nursing standards.

Ability to provide compassionate care to elderly and disabled adults.

Strong communication and interpersonal skills.

Ability to maintain confidentiality and professional boundaries.

Knowledge of infection control and medication administration practices.

Ability to work effectively within an interdisciplinary team.

Strong organizational and documentation skills.

Physical Requirements

Ability to stand, walk, bend, lift, and assist participants throughout the workday.

Ability to lift up to 50 pounds with assistance when necessary.

Ability to respond quickly in emergency situations.

Work Environment

Adult Medical Day Care setting.

Exposure to medical equipment, bodily fluids, and infectious diseases possible.

Fast-paced healthcare environment requiring frequent interaction with participants and staff.
