Responsibilities:

- Provide skilled nursing care to home health patients in accordance with physician-approved care plans
- Monitor and document patient health status, vital signs, and progress toward care goals
- Administer medications, wound care, and other treatments as prescribed
- Educate patients and family members on disease management, medications, and home care techniques
- Collaborate with RNs, physicians, and interdisciplinary care teams to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with state and federal regulations
- Identify and report changes in patient condition to the supervising RN or physician promptly
- Active Florida LPN license in good standing
- Previous home health, long-term care, or clinical nursing experience preferred
- Strong knowledge of wound care, medication administration, and patient assessment
- Excellent communication and interpersonal skills with patients and care teams
- Reliable transportation and valid driver's license for home visits in the Palm Beach Gardens area
- CPR/BLS certification required
- Compassionate, self-motivated, and able to work independently in a home setting
