Job Summary

Responsibilities
- Provide direct, high-quality patient care under RN or provider supervision
- Administer medications and treatments accurately
- Monitor and document patient conditions and report changes promptly
- Support ADLs, wound care, vitals, and routine clinical tasks
- Maintain clear communication with patients, families, and interdisciplinary teams
- Ensure compliance with facility policies and state regulations
Requirements
- Active NH or Compact LPN license
- Current BLS/CPR certification
- Strong clinical skills and reliability
- Experience in long-term care, skilled nursing, or rehab preferred but not required
