About the Role:
As an LPN at Woodland Grove Health and Rehab, you will be responsible for providing high-quality, patient-centered care to our residents. Your duties will include:
- Administering medications and treatments as prescribed by physicians
- Monitoring vital signs and reporting changes to medical staff
- Developing and implementing individualized care plans in collaboration with the interdisciplinary team
- Providing support with activities of daily living (ADLs) such as bathing, dressing, and toileting
- Educating residents and families on care and treatment plans
Responsibilities:
- Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with ADLs
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Maintain accurate and detailed records of resident care and treatment
- Participate in quality improvement initiatives to ensure excellence in patient care
- Maintain a clean and organized environment, adhering to infection control and safety standards

Requirements:
- Current LPN licensure in the state of Florida
- High school diploma or equivalent required
