Licensed Practical Nurse (LPN)

Job Description

Key Responsibilities

• Provide nursing care according to the client’s plan of care and RN direction.
• Administer medications and perform approved treatments and procedures.
• Monitor the client’s condition and promptly report any changes.
• Complete accurate and timely clinical documentation.
• Follow infection-control and safety procedures.
• Educate and support clients and their families.
• Communicate client concerns and care updates to the supervising RN.
• Protect client privacy and follow HIPAA requirements.
• Participate in training and quality-improvement activities.

Qualifications

• Active Virginia or Compact LPN license.
• At least one year of nursing experience preferred.
• Current CPR/BLS certification.
• Experience caring for clients with a tracheostomy and G-tube.
• Home care or community health experience is a plus.
• Ability to recognize and report changes in a client’s condition.
• Strong communication, documentation, and organizational skills.
• Compassionate, dependable, and attentive to client needs.
