Responsibilities:

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Develop and follow individualized patient care plans in coordination with RNs and physicians
• Observe and document patient conditions, progress, and any changes in health status
• Educate patients and family members on care procedures, medications, and health management
• Administer IV therapy, catheter care, and other skilled nursing procedures as ordered
• Communicate effectively with the interdisciplinary care team to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with home health regulations

Requirements:

• Active LPN license in the state of Georgia (required)
• Minimum 1 year of nursing experience, preferably in home health or a clinical setting
• Strong knowledge of skilled nursing procedures and patient care protocols
• Excellent communication, time management, and organizational skills
• Ability to work independently and exercise sound clinical judgment
• Valid driver's license and reliable transportation for home visits
• CPR/BLS certification (required or willingness to obtain)
