Licensed Practical Nurse (LPN) In The Discharge Lobby

Location: Willis Knighton North Department: Discharge Lobby Schedule: Monday through Friday, 8-hour shifts

Position Summary: The Licensed Practical Nurse (LPN) in the Discharge Lobby plays a vital role in ensuring patients leave the hospital with clarity, confidence, and compassion. This position focuses heavily on discharge education, coordination of care, and creating a positive final experience for patients and families. The ideal candidate is detail-oriented, patient-focused, and brings a high level of professionalism and kindness to every interaction.

Key Responsibilities:

• Provide clear, thorough discharge education to patients and families, including medications, follow-up appointments, and care instructions
• Reinforce physician and RN discharge plans to ensure patient understanding and compliance
• Collaborate with nurses, providers, case management, and support staff to facilitate timely and safe discharges
• Answer patient questions and address concerns with patience and empathy
• Ensure all discharge documentation is complete, accurate, and compliant
• Assist with coordination of transportation and post-discharge needs as appropriate
• Maintain a welcoming, calm, and organized discharge environment
• Promote a culture of teamwork, respect, and excellent customer service

Qualifications:

• Current Louisiana LPN license in good standing
• Previous acute care or med-surg experience preferred
• Strong communication and patient education skills
• Ability to multitask and manage patient flow efficiently
• Positive attitude and team-oriented mindset
• Must demonstrate professionalism, compassion, and the ability to work well with all team members
