Baystate Health is seeking a compassionate, patient-focused Licensed Practical Nurse (LPN) to join our Northern Edge Adult/Pediatric team. If you enjoy building lasting relationships with patients, working collaboratively with providers, and delivering exceptional care in a fast-paced outpatient setting, we'd love to hear from you.
As an LPN in our Primary Care practice, you will provide high-quality clinical support to physicians and advanced practice providers while delivering safe, efficient, and patient-centered care to individuals of all ages.
As a member of our clinical team, you will:
- Welcome patients and prepare them for examinations and procedures
- Obtain patient histories, review medications and health maintenance, and record vital signs
- Document patient information accurately in the electronic medical record
- Perform clinical testing, including:
- Bladder scans
- EKG's
- Point-of-care testing (urinalysis, pregnancy testing, blood glucose, HbA1c, throat cultures, etc.)
- Vision and hearing screenings
- Administer medications and immunizations under provider orders and established protocols
- Assist providers during examinations and procedures
- Coordinate patient education, follow-up appointments, referrals, and prior authorizations
- Support chronic disease management, including INR monitoring and controlled substance contract management
- Oversee vaccination scheduling and nurse visits
- Review remote patient monitoring results and communicate changes with providers and RNs
- Maintain accurate patient records and health maintenance registries
- Help ensure efficient patient flow throughout the clinic
- Collaborate with physicians, advanced practice providers, RNs, medical assistants, and administrative staff to deliver outstanding patient care
- Participate in quality improvement initiatives and serve as a mentor to Medical Assistants as appropriate
Qualifications
- Graduate of an accredited Licensed Practical Nursing (LPN) program
- Previous healthcare experience
- Current Massachusetts Licensed Practical Nurse (LPN) license
- Current American Heart Association (AHA) BLS/CPR certification
- Strong communication and interpersonal skills
- Excellent organizational and time management abilities
- Ability to multitask in a busy outpatient environment
- Experience with electronic medical records
- Commitment to providing compassionate, patient-centered care
- Team-oriented with a positive, can-do attitude
Schedule
- Monday–Friday
- 8-4:30pm or 8:30AM – 5:00PM
- No weekends or holidays
