To support a growing team, the full-time Licensed Nurse Supervisor will manage Care Navigators conducting chronic care management and remote patient monitoring programs, fostering patient success and team engagement in a remote setting. Key Responsibilities: Championing a culture of patient-centeredness and support among Care Navigators Empowering Care Navigators by providing tools, resources, and coaching to enhance patient management Monitoring performance and conducting regular one-on-one meetings to ensure team success and retention Required Qualifications: LPN/LVN or greater certification 5+ years of experience in primary or specialty care practice, home care, or chronic care management Proven supervisory experience, preferably in a remote setting High computer literacy and comfort with digital health tools Strong interpersonal skills with a passion for coaching and mentoring staff
