As a critical clinical team member, the remote Comprehensive Health Assessment Nurse Practitioner will conduct thorough, whole-person assessments, develop care plans, and collaborate with care teams to address the needs of members with cancer. Key responsibilities Conduct comprehensive health assessments and evaluations using a telemedicine platform Develop and implement evidence-based care plans in collaboration with Community Health Workers and Oncology Nurse Navigators Review medical records and provide clinical insights to ensure high-quality care for members Required qualifications At least 3 years of Nurse Practitioner experience, including 2 years in adult oncology and 1 year of telehealth experience Compact RN license and unrestricted Advanced Practice license, with a willingness to obtain additional state licenses Experience in completing health risk assessments is preferred Strong organizational skills and ability to manage multiple tasks under pressure Comfort with technology, including video conferencing and electronic health records
