To support a growing telemedicine program, a full-time California Licensed Telehealth Nurse Practitioner specializing in cardiology and heart failure will manage a panel of patients, conduct virtual evaluations, and contribute to care plan development while working remotely from 8:00 AM to 4:30 PM PST, Monday through Friday. Key responsibilities Virtually evaluate patients and create individualized care plans based on specific needs and protocols Respond to alerts from the nursing team regarding patient vitals and symptoms, managing clinical changes within the APP's scope of practice Coordinate with nursing teams to ensure proper medication management and communicate effectively with patients and primary care teams Required qualifications 2+ years as a Board-Certified Nurse Practitioner specializing in heart failure and/or internal medicine Active California nursing license; multiple state licenses preferred Fluency in English and Spanish is highly preferred Previous experience in telehealth and overseeing complex chronic care programs is strongly preferred Ability to work in a HIPAA compliant home environment
