Licensed Nurse Practitioner

To enhance patient access and streamline treatment, the full-time Massachusetts Licensed Nurse Practitioner will provide remote clinical care to patients with neurological conditions and memory disorders, collaborating closely with physicians and the care team. Key responsibilities Deliver direct clinical care via telehealth to patients with neurological and memory disorders Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians Educate patients and families on diagnoses, treatment plans, and disease progression while maintaining accurate clinical documentation Required qualifications Active, unrestricted Nurse Practitioner license in Massachusetts; certification in geriatrics, adult-gerontology, or family practice preferred Minimum of 2 years of clinical experience in neurology, memory care, or dementia care settings Strong clinical judgment and documentation skills Ability to manage a full patient panel independently with physician collaboration Excellent communication skills with patients facing cognitive decline and their caregivers
