Licensed Master Social Worker

Job Description

Seeking a qualified Licensed Master Social Worker (LMSW) to provide social work services, support client needs, and collaborate with healthcare professionals to promote positive outcomes. The ideal candidate will have at least one year of LMSW experience, a Master's degree in Social Work, and a valid New York State LMSW license.

Key Responsibilities:

• Assess clients' social, emotional, and psychosocial needs.
• Provide appropriate social work services, support, and referrals based on client needs.
• Coordinate care and connect clients with relevant social services and community resources.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Maintain accurate and timely documentation of assessments, interventions, and services provided.
• Communicate effectively with clients, families, and healthcare professionals.
• Support continuity of care through appropriate follow-up and resource coordination.
• Maintain professional standards, confidentiality, and compliance with applicable regulations.

Qualifications:

• Experience: Minimum 1 year of LMSW experience.
• Education: Master's degree in Social Work (MSW) required.
• Licensure: Valid Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED).
• License Verification: Primary source verification required.
• Required Skill: Social services.
• Strong communication, interpersonal, and organizational skills.
• Professional judgment and the ability to work collaboratively with interdisciplinary teams.

Important Details:

• 14-week contract position (extensions possible)
• Shift is 7 hours/day, Mon - Fri (Timing between 9 AM - 5 PM), 35 hrs/week
• Location is Bronx, NY 10451
• Local Pay Range: $60 - $63 per hour
• Travel Pay Range: $2,450 - $2,500 per week
