Licensed Master Social Worker

Seeking a Social Worker to provide comprehensive psychosocial support to hospitalized patients and their families. The role involves psychosocial assessments, treatment planning, patient and family interventions, discharge planning, community referrals, and collaboration with interdisciplinary teams to ensure timely and effective patient care.

Key Responsibilities:

• Screen patients for social work needs within 24 hours of admission.
• Complete initial psychosocial assessments within 72 hours and develop individualized treatment plans.
• Provide ongoing psychosocial assessments and interventions based on patient and family needs.
• Develop safe, timely, and effective discharge plans.
• Coordinate discharge planning with interdisciplinary teams, patients, families, and community agencies.
• Refer patients and families to appropriate community resources to support continuity of care.
• Maintain accurate and timely documentation according to professional and departmental standards.
• Participate in discharge planning rounds, staff meetings, supervision, and professional development activities.
• Maintain monthly statistics and share relevant community resources with staff.
• Apply social work ethics, professional standards, and organizational policies in daily practice.
• Perform other related duties as assigned.

Qualifications:

• Master’s Degree in Social Work (MSW) required.
• New York State Licensed Master Social Worker (LMSW) required.
• Previous inpatient hospital/Med-Surg social work experience preferred.
• Strong interviewing, assessment, communication, and decision-making skills.
• Ability to exercise sound judgment and respond effectively in a hospital environment.
• Bilingual in Chinese or Spanish preferred.

Important Details:

• 8-week contract position (extensions possible)
• Shift is 7.5 hrs/day (8:30 AM–4:30 PM), 37.5 hrs/week
• Location is New York, NY 10038
• Local Pay Range: $54 - $58 per hour
• Pay Range for travelers: $2,340 - $2,380 per week
