Licensed Hospice Journey Counselor

To support patients and families during end-of-life planning, the full-time Licensed Hospice Journey Counselor will provide education and assistance in transitioning to hospice care while managing patient referrals and maintaining communication with healthcare teams remotely. Key responsibilities Manage patient census and referrals, ensuring eligibility for hospice services Conduct initial and follow-up calls to new Journey referrals and schedule home visits Document all patient interactions and maintain accurate Journey patient charts Required qualifications High school diploma or GED Prior home healthcare or relevant experience in the healthcare industry preferred Understanding of the hospice philosophy preferred Must hold and maintain a professional license, certificate, or registration as applicable
