We are seeking a Licensed Clinical Social Worker (LCSW) to provide comprehensive mental and behavioral health services within an Integrated Behavioral Health (IBH) model. This role delivers patient-centered care through mental health and substance use assessments, short-term evidence-based therapy, crisis intervention, and care coordination in collaboration with multidisciplinary teams.
Key Responsibilities:
- Conduct comprehensive mental health and substance use assessments.
- Perform integrated behavioral health screenings in a primary care setting.
- Provide short-term, solution-focused therapy using evidence-based approaches (CBT, Motivational Interviewing, Trauma-Informed Care).
- Develop and implement individualized treatment plans.
- Provide individual and group therapy.
- Conduct crisis intervention and risk assessments.
- Coordinate referrals to specialty mental health, substance use treatment, and community resources.
- Collaborate with care teams within the IBH model.
- Maintain accurate, compliant clinical documentation.
- Deliver culturally competent care and educate patients/families.
- Master's degree in Social Work (MSW) from a CSWE-accredited institution.
- Active California LCSW license.
- Minimum 2-3 years of clinical experience required.
- Experience in integrated/community healthcare preferred.
- Strong background in psychosocial assessments, treatment planning, crisis intervention, and substance misuse treatment.
- Familiarity with DSM-5 criteria and behavioral health insurance guidelines.
- Experience working with adolescents/minors preferred.
