In this resident-focused, mission-driven role, you will provide counseling, psychosocial assessments, care planning, and crisis intervention for older adults across independent living, assisted living, memory care, and skilled nursing. You’ll collaborate closely with interdisciplinary teams, advocate for residents and families, coordinate transitions of care, and connect residents with community resources while contributing to a culture of respect, inclusion, and professional growth.
