Overview

Senior Care Therapy (SCT) is seeking full-time or part-time Licensed Clinical Social Workers to provide in-person psychotherapy services to the geriatric population in Skilled Nursing Facilities, including sub-acute, long-term care, and assisted livings throughout NJ, NY, PA, and MD. SCT handles administrative responsibilities such as billing, insurance, credentialing, and pre-certifications so clinicians can focus on delivering clinical care. We aim to support residents and care teams with patient-centered psychology services and to stay at the forefront of regulations, needs, and trends.

Key Responsibilities

• Conduct individual and group psychotherapy sessions for patients in subacute or skilled nursing facilities
• Assess, diagnose, and treat a range of emotional and behavioral health conditions (e.g., depression, anxiety, adjustment disorders, cognitive decline)
• Develop and implement evidence-based treatment plans tailored to each patient
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care
• Maintain accurate and timely clinical documentation in accordance with regulatory standards using the clinician-designed electronic health record system
• Monitor patient progress and adjust treatment plans as needed

Requirements

• Active and unrestricted Licensed Clinical Social Worker license in the state you are applying for
• Effective oral and written English communication
• Basic proficiency with technology and electronic health records (EHR)
• Strong organizational and documentation skills with attention to regulatory compliance
• Ability to provide in-person services at assigned facilities, up to 45 minutes
• Ability to walk, stand, and move between patient rooms and offices within the facility
• Full vaccination and booster status may be required in some facilities

Preferred Qualifications

• Previous clinical experience in subacute, long-term care, or geriatric healthcare
• Experience treating anxiety, depression, and adjustment disorders
• Psych or addictions experience is a plus
• Excellent time management and organizational skills
