Licensed Clinical Social Worker Advocate

To support a growing practice, the Licensed Clinical Social Worker Advocate will provide psychosocial assessments, resource navigation, and behavioral health support to members, working remotely on a 1099 contract basis with flexible scheduling options. Key responsibilities Complete psychosocial and social determinants of health assessments to identify barriers affecting member wellbeing Connect members to appropriate community resources and benefits, ensuring follow-up on resource effectiveness Provide brief, solution-focused support and coordinate referrals to behavioral health providers when necessary Required qualifications Active, unrestricted LCSW license in at least one U.S. state Master of Social Work (MSW) from a CSWE-accredited program 2+ years of post-master's experience in medical, community, geriatric, or healthcare social work Strong knowledge of public benefits and community resources Self-directed with the ability to manage a member load and schedule independently
