Seeking a full-time Licensed Clinical Pharmacist fluent in Cantonese/Mandarin to work remotely, who will manage chronic medication therapy, conduct comprehensive medication reviews, and collaborate with interdisciplinary teams to enhance patient care and medication adherence. Key responsibilities Provide personalized care plans and educational materials to help patients manage chronic conditions Conduct comprehensive medication reviews and resolve drug therapy issues to optimize patient outcomes Drive quality pharmacy-related clinical measures to achieve "5-Star" performance and address therapy gaps Required qualifications 3 or more years of clinical experience in direct patient care State licensure in good standing in at least 5 states, including Alabama, Florida, Georgia, or others as needed Residence in the Mountain, Pacific, or Hawaiian time zones or ability to support work during those hours Fluent proficiency in Cantonese/Mandarin Experience with collaborative pharmacy practice agreements preferred
