Focusing on Transitions of Care support, the full-time Licensed Behavioral Health Case Manager will provide short-term interventions to members transitioning to community-based behavioral health services, working remotely while preventing avoidable hospitalizations. Key responsibilities Perform comprehensive psychosocial assessments via phone and video with members and caregivers Conduct clinical social work interventions to enhance program effectiveness and address social determinants of health Collaborate with interdisciplinary teams to ensure cohesive, patient-centered care for high-needs patients Required qualifications Master's degree in Social Work with an active clinical license (LCSW, LMSW, or comparable) Minimum of 5 years of experience in behavioral health or care management Competence in performing comprehensive psychosocial assessments remotely Able to manage a fully remote work schedule, M-F, 8:30am-5:00pm PST or MST Willingness to obtain additional state licenses as needed
