License Practical Nurse (LPN)

JOB TITLE: Licensed Practical Nurse (LPN) Per Diem/Temp

DEPARTMENT: Residential/DHS

REPORTS TO: Program Coordinator

FLSA STATUS: Non-Exempt

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:

• LPN must supervise medication and ensure that consumers are compliant with

medication regimen. Prepare medication setup for consumers.

• Monitor medication log and consumer medication sheets.

• Work closely with pharmacies and the psychiatrist to ensure consumers obtain prescriptions

that are filled in a timely manner.

• Must ensure that documentation is accurate and provide feedback and/or concerns to

psychiatrist and other staff.

• Complete health assessments for new clients and annual assessments for current

clients (e.g., blood pressure test) for each consumer and enter all information into the

client electronic health record system AWARDS.

• Candidate must counsel clients about medication non-compliance and other health

related matters as needed.

• Other duties reasonably relevant to the position may be assigned.

QUALIFICATIONS:

Must possess a Nursing Degree and an NYS-licensed Practical Nurse (LPN) license is required; at

least two years of mental health experience and experience with adult consumers with co-occurring disorders. Candidate must have good interpersonal skills; excellent written and verbal

communication skills. Proficient in Microsoft Office applications, including Word, Excel, and

knowledge of the OHS CARES system and AWARDS is a plus.

SCHEDULE:

Flexible schedule; days may vary. Minimum of 24hrs/week.
