Lead Primary Care Physician — Patient-Centered Team Care

Millennium Physician Group is seeking a Physician, General / Primary Care to provide comprehensive primary care across the lifespan within its ambulatory practice. You will evaluate, diagnose, and manage acute illnesses, chronic diseases, and preventive services using evidence-based medicine in a patient-centered team environment.

You will collaborate with APPs and care teams, participate in quality initiatives, and lead care coordination efforts to improve outcomes and support value-based care.
