Lead Primary Care Physician

ChenMed, a physician-led and mission-driven primary care organization, is seeking a Lead, Primary Care Physician (PCP) to join their rapidly expanding team. This role is the initial step in the clinical leadership track, focusing on delivering affordable VIP care to seniors and transforming healthcare for a high-need population. The Lead PCP will be accountable for patient outcomes, act as a health coach, and lead care teams to achieve performance goals.

Practice Info

• Physician-led and mission-driven primary care organization
• Focus on full-risk Medicare Advantage providers
• Panel of at least 360 patients per Lead PCP
• Care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator
• Engages with transitional care team for patients unable to come to the office (in hospital, SNF, LTC, or homebound)

Responsibilities

• Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient
• Takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed
• Engages with the hospitalist whenever one of their patients is in the hospital
• Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs
• Leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office
• Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, as well as supports Center Manager/Center General Manager if the center clinical leader and/or market clinical leader is not available
• Plays an active role in the management of their center and helps cover for other providers who may be out for various reasons
• Takes an active role as needed in recruiting patients for the center and recruiting and interviewing additional providers for the company

Compensation

• $111,140 - $158,771 Salary
• Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors
• This position may also be eligible for a bonuses or commissions

Shift & Schedule

• 16-18 appointment slots per day at a 0.9 FTE PCP schedule / 0.1 FTE dedicated leadership time (each new patient count for 2 slots, follow-up patients, typical 1.0 PCP FTE slots are 18-21)

Requirements

• Demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient
• Acts as a health coach, helping patients set short and long-term health goals
• Simplifies and prioritizes appropriately for patient behavior change and leading care teams
• Demonstrates leadership by leading Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, and supporting Center Manager/Center General Manager as needed
• Ability to function both independently and in collaboration with other health care professionals
• Adherence to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures
• Available and Accessible for patients in order to build trust, including being open for walk-in visits and answering phone calls and messages in a timely manner
• Remains updated on evidence-based medicine, clinical, nutritional, and lifestyle-based interventions
• Willingness to continuously work to develop and improve leadership skills
• Willingness to work with care teams towards achieving high quality outcomes and quality measures
