LTFC is seeking a compassionate and detail-oriented Lead Medical Coordinator to join our dedicated team. This role plays a vital part in supporting the health and well-being of children and families through direct care coordination and community-based services.
Location and Schedule
Location: 843 Whalley Ave, New Haven, CT
Schedule: 8:30 am - 5:00 pm, this position requires flexibility and has varied hours

Key Responsibilities
- Coordinate medical care and services for children in our programs
- Conduct home visits to assess and support client needs
- Escort children to and from medical appointments
- Collaborate with healthcare providers, case managers, and families
- Maintain accurate documentation and ensure compliance with care standards
Required Education and Experience
- Licensed Registered Nurse (RN) or bachelor’s degree in a health-related field (e.g., Physician Assistant, Nurse Practitioner)
- Minimum of 1 year of experience in public health or healthcare settings
- Demonstrated experience in care coordination, health administration, or direct clinical care
- Supervisory experience and licensure to provide clinical services in the state of employment
- Bilingual proficiency in English and Spanish
- Experience conducting home visits and supporting children during medical transitions
