M&H Housecalls invites an experienced Home Visit APRN or PA to lead clinical care in patients' homes from Naples, FL. You will guide the care team through transitions from acute care to home, directing multidisciplinary involvement and ensuring continuity.
Expect 6-10 home visits daily, with emphasis on preventing ED visits and hospital admissions, and stepping in as interim provider where no PCP exists or as a co-manager with the patient’s PCP.
